The following templates are used internally by the Office of Risk Assessment while reviewing material transactions. Templates provide basic review criteria which assists in transaction approval / disapproval.

These templates have been put on the website with the intent to assist companies in providing complete material transactions filings to the Department of Insurance, Office of Risk Assessment. Completion of a material transaction template by the company prior to filing with the Ohio Department of Insurance will facilitate and enhance the transaction review process.

In addition, the company should consider adding additional supplementary schedules to their filings that will expedite the review process. For example, a flowchart (box diagram) explaining the purpose and flow of the transaction can be invaluable to OFRS during the review process. The company should use a “plain English” writing style while preparing disclosures, agreements or contracts. Consider adding a term sheet to the filing, as it will also benefit the department in understanding the transaction being proposed.
